Pre Season Notice

As the Varsity Team will be training in Hawaii the week before school starts, the regular High School Water polo season will begin on the first day of school at 3:15 in the afternoon. Please make sure to have the following completed and signed by the required persons.

1. A pre-participation Physical form (2pages) that has been completed and signed by you, your, parents and most importantly your Doctor.

2. The District Insurance information and Release form this also must be signed by you, your parents, and your Doctor.

3. The High School Code of conduct and Steroid warning form. this must be read and signed by you and your Parents.

4. The Monta Vista Athletic Boosters form. This should be completed by your parents and a check for the appropriate amount attached.

All of these are available on the Monta Vista Athletic Web site.

You will also need to complete a form for the team suit and t-shirt.

Please down load all of the forms and have them completed by the first day of school so that you can begin practice immediately. Please make sure to fill all forms out completely, as if any one of these forms is incomplete you will not be able to participate until they have been finished.
